Free flap Auricular reconstruction

In cases where the designated area for transplant is covered by a scar tissue caused by burns, accident or repeated surgery, or by supplemental radiation therapy for cancer that subsequently lead to a deficiency in blood supply to the area, a more complicated surgery for reconstructing the pinna is required
.
In such cases, a more complicated surgery is required where the cartilage of the new pinna, which was meticulously formed under the skin in a different body part, is grafted as described in (reconstructive surgery for the pinna and the ear canal).

In a later stage, an additional surgery is performed for transferring the new pinna to the designated area by microvascular surgery in which the blood vessels are joined to the blood vessels of the neck.

במקרים כגון אלו נדרש ניתוח מורכב יותר בו משתילים את סחוסי האפרכסת החדשה כפי שתואר ב (ניתוח שחזור אפרכסת ותעלת אוזן) שעוצבה בקפידה מתחת לעור באיזור אחר בגוף.
 
בשלב מאוחר יותר מבצעים ניתוח נוסף בו מעבירים את האפרכסת החדשה לאיזור המיועד בניתוח מיקרווסקולרי בו מחוברים כלי הדם לכלי דם בצוואר
